Bring medical records including proof of prior immunizations or vaccination history and in some cases you may have prior chest X-rays reports or treatment history regarding Tuberculosis or other communicable/infectious diseases.
At your visit you will fill out some forms or you may download the forms and prepare ahead:
If your skin test result is negative and your immunizations are up to date you will receive your completed I-693 form without needing an X-ray.
If your blood test shows lack of immunity, or if you lack vaccine records, then immunizations will be recommended.
Your I-693 form will be completed after the recommendations have been successfully completed.
Note: The results of the medical examination are generally valid for only 12 months. Applicants should schedule the medical examination as close as possible to the time they file for adjustment of status, but provide for sufficient time for the performance of laboratory testing or additional testing required under CDC’s Technical Instructions, which at times can take a few days to 6 months if treatment for exposure to TB is necessary.

She was professional and very kind, and made me feel very welcomed as if I were at my relatives home.
The day I picked up my completed document from her, I had very bad cold. I tried to make an appointment with my primary doctor at Jefferson for emergency visit that morning, and was told there was no space for me that day.
I told that to Dr. Steel when I was at her office, then she just made a call to them and manage to make an appointment for me!! Thanks to her, I got to see my internist that day, got the med I needed.
She’s amazing. Highly recommended!!!!
