How to prepare for immigration physical:
Bring Government Issued photo identification: passport or driver license
Bring medical records including proof of prior immunizations or vaccination history and in some cases you may have prior chest X-rays reports or treatment history regarding Tuberculosis or other communicable/infectious diseases.
What to expect:
At your visit you will fill out some forms or you may download the forms and prepare ahead:
You will be asked your medical history, a physical examination will be performed, a skin test for Tuberculosis performed and blood work will be ordered.  You will return in 2-3 days to have your skin test results examined.
If your skin test result is negative and your immunizations are up to date you will receive your completed I-693 form without needing an X-ray.
If your skin test result is positive, you will need to get an X-ray and treatment for Tuberculosis may or may not be recommended.
If your blood test is positive for signs of disease, treatment will be recommended.
If your blood test shows lack of immunity, or if you lack vaccine records, then immunizations will be recommended.

Your I-693 form will be completed after the recommendations have been successfully completed.

Note: The results of the medical examination are generally valid for only 12 months. Applicants should schedule the medical examination as close as possible to the time they file for adjustment of status, but provide for sufficient time for the performance of laboratory testing or additional testing required under CDC’s Technical Instructions, which at times can take a few days to 6 months if treatment for exposure to TB is necessary.

I absolutely recommend Dr. Steele for your physical. She was so professional, dynamic, fast and knowledgeable! My physical was done in a matter of days and during those stressful days she offered support and guidance. She is amazing!!!!Quisiera recomendar a la Doctora Steel para tu physical. I found her in the list of doctors who handle medical checkup for Green Card. I called other 3 doctors and she was the most reasonable, and accessible( middle of the center city of Philadelphia.)When I got there I was gladly surprised how beautiful and comfortable her office was, and it shows how she is.
She was professional and very kind, and made me feel very welcomed as if I were at my relatives home.

The day I picked up my completed document from her, I had very bad cold. I tried to make an appointment with my primary doctor at Jefferson for emergency visit that morning, and was told there was no space for me that day.
I told that to Dr. Steel when I was at her office, then she just made a call to them and manage to make an appointment for me!! Thanks to her, I got to see my internist that day, got the med I needed.
She’s amazing. Highly recommended!!!!

指定医のリストから何件か問い合わせたのですが、こち らの先生が一番低価格でした。
オフィスはとても便利なロケーションで、きれいです。
先生はとてもやさしく、日本の小さな診療所のように親 身に対応してくれます。

I have been so lucky to find Dr. Steele. She was extremely professional, honest and gave clear and precise instructions to follow, expediting the process and making it really easy.
